Along with the forms below, we will need a copy of your child’s birth certificate and original immunization card.
Please click on the link above to be taken directly to our online registration.  Once we have this information we will email you to let you know of any other information or forms that we need from you or we will direct you here to our website to print out these forms such as the ones below.  You may then print the forms and bring it in to us with registration payment, email it to deboraheddie@gsbc.onmicrosoft.com, or fax it to 251-660-1097.  You may pay registration by dropping off check or cash payment or for your convenience you may pay online by going to  http://www.governmentstreet.org, click the giving tab and then select summer camp registration.  Please call if you have any questions. 
Releases & Authorizations Page    This form will need to be initialed and signed.
Just like last summer, you will be given the ability to authorize recurring payments through your checking/savings account or debit/credit card.  The following link will give you access to this authorization form.  However, as you will see on the form below, you will be able to set up your own recurring payments or make payments through our new program, Headmaster Online.  Once you are registered for summer camp, you will be emailed a link with instructions to set up Headmaster online.   If you do not wish to sign up for automatic payments, you may check the box at the bottom of this authorization form letting us know that you will be making cash or check payments in advance of attendance each week.  However, each camper must have a secondary form of payment on file that will be charged if payment has not been received by close of business on Thursday.
